A google or Microsoft form is basically a form that you can use as a quiz or just a way to get information. It isn't too hard to make and very easy for students or parents to fill out. These questions can be multiple choice, a poll, text, or a date. As the creator of the form you can make questions required or not. When you see the responses it shows you individual responses as well as a whole with an average of how many answered what.
This individual Microsoft form that I made for my virtual co-op was for the parents/guardians to fill out for the virtual open house with the teacher. There is a spot for their name as well as the child's name. Then there are options of times for them to attend the open house starting at 5:30 and going till 7:15 each within 15 minutes apart. This Microsoft form will help the teacher see when each parent is available and help plan a time for them to meet virtually for their open house. This goes along with the Danielson Framework 4c. Communicating with families. Where the teacher is involving them into the classroom to get to know them and introduce them to what is going on throughout the year. A google or Microsoft form in general would probably fall under Danielson's Framework 1d. Demonstrating Knowledge of resources. This is because a form is a great resource to use inside or outside a classroom so being able to demonstrate knowledge through the form could be very useful.
I'll probably use either of these types of forms in my future classroom not as much for the students since my future classroom will be more younger kids but more for the parents as a way to communicate since I will be working with younger ones. This will help me gather information from them along with sending out information and communicating with them. Although if classes in my future school are more an electronic base system then I could do forms using more pictures than words for the answers.
